Working in collaboration with the Suffolk County Department of Health Services, the Diabetes Team of health professionals, including RD, RN and community advocates, are working together to fight diabetes.
The program goal is to help participants make behavior change, through knowledge and skills for effective DIABETES SELF-MANAGEMENT. This will lead to prevention of chronic complications and costs.
