2008 Registration Information

Registration forms are available online in two ways:

Other Required Forms:

The following forms are required of ALL Campers, and must be submitted to Hidden Valley 4-H Camp, 323 Owego St., Unit #5, Montour Falls, NY 14865 no later than July 1, 2008.

RESIDENTIAL CAMPERS:

• Camper Health History Form 1 (Complete this form and make a copy. Send the original to camp and take the copy, along with Form 2 below, to your Health Care Provider's Office).
• Camper Health Care Recommendations by Licensed Medical Personnel Form 2
• Activities Selection Form 2008
• Camper Code of Conduct
• Acknowledgement of Risk Form

DAY CAMPERS:

• Day Campers Health Form
• Activities Selection Form 2008
• Camper Code of Conduct
• Acknowledgement of Risk Form

Registrations are accepted until each camp week is full, but to qualify for the Early Bird Discount, registration must be paid in full no later than April 1, 2008.

Registrations must be accompanied by a check or money order covering at least a $75/week non-refundable deposit.

ALL registration forms, health forms, and payment of all fees must be completed and received at the Camp Office no later than July 1, 2008.

Sorry, we do not have the ability to accept credit card payments.

Registration forms for campers receiving camperships or other funding must be accompanied by either a check from that agency or confirmation IN WRITING from the funding agency of the amount being paid by campership. More campership information is available here.
